Monday - Friday
7:00am-5:30pm - Cath Lab at the Hospital except for the below days:
Every Wednesday 8:00am -5:00pm - In- Office Clinic
Every 2nd Friday of the month 8:00am -5:00pm - Telemedicine Clinic
Every 4th Friday of the month 8:00am -5:00pm - In-Office Clinic (First Patient at 9:00am) night beeper call on Fridays
Patients per day: Clinic 20-22 patients
Patient Population/Age: adult
Department Description: Cath Lab will be hospital consults 3-4 per day, plus EP cases 4-6 a day depending on case length
EMR: Cerner
Hospital Bed size: 268
Special Procedures: cardiac electrophysiology; specifically, heart rhythm disturbances, Ablations - must show 100 EP cases in the last 2 years in case logs with emphasis on ablations
Skills Required: Ablations
Practice Setting: Hospital
In-Office Clinic
BC or BE required
License Required: AZ Licensed
Willing to License
DEA: AZ registered
CSR: not required
ACLS Required
Minimum Experience Required: 1 year
Credentialing Timeframe: 30 days
