Job Title: Home Health Physical Therapist

Join our home health team as a Physical Therapist in a 13•week contract role, working a 40•hour week with potential weekend shifts.

Responsibilities:
• Deliver home health physical therapy services
• Document patient progress accurately and timely
• Manage a caseload of 20•25 patients weekly

Qualifications:
• Valid PT License
• BLS Certification
• 2+ years of physical therapy experience
• Home health and OASIS experience
• Maximum 1•year employment gap

Additional Information:
• Reside within 30 minutes of service area
• No on•call duties
• Pre•approved time off
• Weekly in•person office attendance
• Complete Homecare Homebase documentation at patient's home
