Family Practice/Primary Care Nurse Practitioner

Job Summary:
In this role, you ll assess, create, and oversee treatment plans for residents in senior living communities and those receiving in-home wound care. As a wound care provider, you ll diagnose and treat various types of wounds, perform lab tests, make referrals, and carry out bedside procedures based on your expertise and available tools. You ll also collaborate with internal and external healthcare providers to ensure comprehensive care for patients in both home and facility settings.
