Family Practice/Primary Care Nurse Practitioner

SSM Health is actively searching for a dedicated and experienced Family Nurse Practitioner to become an integral part of our dynamic healthcare team in our outpatient clinic in Tipton, MO. In this role, you will play a vital role in delivering comprehensive primary care services with a focus on health promotion and disease prevention.

Responsibilities:

• Conduct thorough patient assessments, including medical history and physical examinations.
• Diagnose and treat a variety of acute and chronic illnesses, collaborating with physicians and other healthcare professionals as needed.
• Develop and implement patient care plans, ensuring individualized and evidence-based care.
• Prescribe medications and treatments in accordance with state regulations and practice guidelines.
• Provide patient education on preventive healthcare measures, lifestyle modifications, and treatment plans.
• Collaborate with a multidisciplinary healthcare team to optimize patient outcomes and continuity of care.

Qualifications:

• Graduate of nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program
• Current or ability to obtain PA/NP licensure in Missouri
• Experience in primary care or family medicine is preferred.
• Strong interpersonal and communication skills, fostering positive patient-provider relationships.
• Commitment to staying current with advancements in healthcare and continuously improving clinical skills.
