Senior Transitions of Care Associate - (Emergency Department)

In this role, you will support safe, timely transitions of care for patients moving from the Emergency Department to inpatient units, outpatient services, or home. You will partner with ED pharmacists, nurses, and other care team members to ensure patients have the medications, instructions, and follow-up they need so their treatment continues smoothly and without interruption.

Your work will center on real-time support within the ED -helping prepare patients for discharge, coordinating medication access, facilitating care transitions, and tracking movement across care settings to maintain continuity of therapy.

Role Highlights

  • Coordinate transitions of care for ED patients, ensuring medications and therapy plans follow the patient to their next care setting.

  • Support ED discharges by assisting with medication procurement, counseling coordination, and follow-up arrangements.

  • Partner with pharmacists, nurses, case management, and pharmacy financial counselors to reduce barriers to discharge.

  • Assist with medication histories and support ED pharmacy workflows as needed.

  • Follow policies and procedures that promote patient safety, efficiency, and high-quality care during rapid patient throughput.

Shift

  • Full Time and benefits-eligible (0.9 FTE / 72 hours per pay period)

  • Third Shift, 9:00pm - 7:00am

  • 7 on 7 off schedule rotation- Every other weekend, Will work any holidays that fall on assigned week.

    Duties: Senior Transition of Care Associate ED


    Facilitate and coordinate patient care transitions at admission, transfer, and discharge from the facility. Utilize available electronic resources, patient interview, and phone follow up to ensure an accurate medication history is available for patients at admission or upon presentation to the emergency department. Work in coordination with and under the supervision of a registered pharmacist to ensure accurate medication reconciliation is performed per policy MM07. Review transfer orders and ensure necessary medications have been transferred with the patient. Collaborate with inpatient pharmacists, inpatient nursing, pharmacy financial counselors, and the meds to beds outpatient pharmacy staff to assist in ensuring patient discharge needs are met in a timely manner.

    Required Qualifications: Senior Transition of Care Associate ED

    * Minimum of three years of experience in an inpatient or outpatient pharmacy or other applicable health care setting required.
    * High school diploma or equivalent required.
    * Effective communication skills, initiative, teamwork and flexibility required.
    * Registered as a pharmacy technician with the State of Nebraska Board of Pharmacy prior to start date required.
    * Pharmacy Technician Certification within twelve months of registration date required.
    * Basic Life Support certification within 90 days of transfer or hire required.

    Preferred Qualifications: Senior Transition of Care Associate ED

    * Thorough understanding of available electronic resources to obtain medication history information preferred.
    * Thorough understanding of medical terminology preferred.
    * Thorough knowledge of top 100 medications preferred.
    * Associates or bachelor's degree preferred.
    * Proficient in Microsoft Office preferred.
