Position Summary: The Pharmacist, is responsible for providing medication therapy management services in collaboration with other providers to assure that drug therapy is safe, effective, efficient and cost-effective.

Position Requirements

Education: Doctor of Pharmacy required.

Licensure: Licensed Pharmacist in the state of practice required.

Certification: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) required.

Experience: 3+ years' experience in a hospital or retail pharmacy setting required.


Required Knowledge/Skills/Abilities

  • Requires skills in the operation and use of pharmacy techniques and supporting equipment and materials, communication, training and orienting new staff.
  • Requires the ability to conduct meetings and ensure information is given to the appropriate staff, communicate both orally and in writing to staff.
  • Requires the ability to identify problems and offer well thought out solutions to those problems, write and communicate proposals regarding the management of the dept. or other nursing problems, learn and adapt to change and to communicate those changes in a supportive manner to all staff.

Essential Job Functions

  • Pharmacy services provided may include interdisciplinary patient care rounds, medication reconciliation, medication therapy monitoring, medication orders management, patient education, drug information, drug dosing, and various scholarly activities.
  • The pharmacist ensures that patient care is maintained through compliance with the department's policies, rules and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies.
  • The Pharmacist performs both clinical and distributive tasks necessary to ensure that patients' medication therapy is safe, effective, timely and efficient.
  • The pharmacist acts independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines with support from the Pharmacy Manager.
  • Assists the pharmacy and hospital staff with responsibilities in accordance with the organization's mission, policy, and any applicable laws.
  • Monitors activities of the Pharmacy Department and takes appropriate action as necessary to ensure efficient and effective operation.
    • Activities may include Retail pharmacy tasks such as interpreting prescriptions, entering McKesson Enterprise system, prescription verification, prescription counseling and delivery.
    • Activities may include Inpatient pharmacy tasks such as order interpretation and verification, home medication reconciliation, DUR checking, nursing education, order delivery and stocking.
    • Activities may include supervision of Pharmacy technicians.
  • Works closely with the nursing and medical staffs and other clinical department managers in addressing Pharmacy Department issues as they are related to the services provided to all patients.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical errors. Performs inpatient duties that include but are not limited to:
  • Maintains regulatory requirements, including all state, federal and Joint Commission regulations.
  • Employee is responsible for all other duties as assigned for which competency has been demonstrated.

Physical requirements: This position is classified as Light Work in the Dictionary of Occupational Titles, requiring the exertion of up to 20 pounds of force frequently, and/or a negligible amount of force constantly (66% of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work.
