Position Description

Job Requirements:

  • An associates degree of bachelor’s degree in Nursing.
  • At least three years experience in adult hospital nursing.
  • Experience educating patients in areas such as: Oncology, Cardiology, Diabetes, Domestic Violence.
  • Communication skills in presenting health topics to individuals and groups.
  • Work with “at-risk” or “under-served populations” and has the ability to work with physicians in the community regarding identified areas of community health needs.
  • Organization and time management skills.
  • Ability to develop creative solutions in working with “at risk” or “under-served populations”.
  • Computer skills in Excel, Word and patient care software.
  • Preference for individuals with Spanish speaking skills.

 

Job Duties:

  • Assist the Community Benefits Manager in the development of the Community Benefit Plan.
  • Participate in community benefit sponsored events and programs.
  • Work with physicians on the content of the various community benefit programs.
  • Work in cooperation with Marketing to develop and internal and external communication plan outlining the activities and results of the Community Benefit Program.
  • Facilitate the development of new educational, screening and care management programs identified in the Community Benefit Plan.
  • Organize and report activities concerning community benefits on the IRS 990 H form and other interested stakeholders.
  • Participates in mandatory in-service and/or CE programs as mandated by policies and procedures/external agencies and as directed by Supervisor.
  • Follows and understand the mission, vision, core values, Employee Standards of Behavior and policies/procedures.
  • Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.
  • Follows and understands the mission, vision, core values, Employee Standards of Behavior and company policies/procedures.
