Job Requirements:
- An associates degree of bachelor’s degree in Nursing.
- At least three years experience in adult hospital nursing.
- Experience educating patients in areas such as: Oncology, Cardiology, Diabetes, Domestic Violence.
- Communication skills in presenting health topics to individuals and groups.
- Work with “at-risk” or “under-served populations” and has the ability to work with physicians in the community regarding identified areas of community health needs.
- Organization and time management skills.
- Ability to develop creative solutions in working with “at risk” or “under-served populations”.
- Computer skills in Excel, Word and patient care software.
- Preference for individuals with Spanish speaking skills.
Job Duties:
- Assist the Community Benefits Manager in the development of the Community Benefit Plan.
- Participate in community benefit sponsored events and programs.
- Work with physicians on the content of the various community benefit programs.
- Work in cooperation with Marketing to develop and internal and external communication plan outlining the activities and results of the Community Benefit Program.
- Facilitate the development of new educational, screening and care management programs identified in the Community Benefit Plan.
- Organize and report activities concerning community benefits on the IRS 990 H form and other interested stakeholders.
- Participates in mandatory in-service and/or CE programs as mandated by policies and procedures/external agencies and as directed by Supervisor.
- Follows and understand the mission, vision, core values, Employee Standards of Behavior and policies/procedures.
- Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.
